Who covers the cost?
Medicare will cover the cost of home health care services for patients over 65 who meet eligibility requirements. Private insurance companies, managed-care organizations, and various other payers also cover home health services.
What are the benefits?
Home health care – provided in the comfort and privacy of a patient’s home – can benefit those who need one-on-one instruction to help better manage their condition, symptoms, and recovery. Home health may be indicated if a patient:
- Has a new diagnosis or an exacerbation of an existing diagnosis
- Has a new medication
- Requires teaching on a disease process or medication
- Is at risk for or experiencing complications requiring labs
- Requires other skilled nursing services (wound care, teaching, observation, and assessment)
- Requires frequent follow-up from a physician’s office
- Has frequent re-hospitalizations
- Contacts a physician’s office frequently
- Exhibits behaviors that indicate he or she is at risk for falls
- Needs physical therapy, occupational therapy, or speech therapy, due to: mobility problems, bowel or bladder incontinence, vision problems, ADL needs, and new DME training
What types of services are provided?
Home health services are provided by professional nurses and therapists. This team of professionals works in consultation with a patient’s physician to provide a full suite of home health services and programs designed to achieve the best possible outcome.
Services may include:
- Medication management and education
- Education on diseases or conditions
- Dietary and nutrition evaluation
- Treatment progress monitoring
- Help with mobility challenges and/or fall risk – and minimization of risk
- Help managing chronic conditions (diabetes, COPD, etc.)
- Other programs designed to hasten recovery and improve quality of life
